The mission of Giving Hearts, LLC is to empower individuals to attain autonomy by teaching them to realize goals and meet their needs in a healthy, professional, responsible, and productive manner.
Men and women at least 18 years of age with a primary diagnosis of Intellectual Disabilities ranging from mild to severe.
Must possess or be eligible for ID Medicaid Waiver.
At least two deficits documented at the adaptive skill level in areas to include: Health, communication, task learning skills, personal/self-care, mobility, social skills, or community and home living skills.
Other disabilities such as hearing loss, spastic cerebral palsy, vision impairment, epilepsy, diabetes, autism, down syndrome, etc.
Individuals with mental health disorders accepted
Comprehensive, Initial, Ongoing and Annually
Multi-disciplinary Team Approach for increase service options
Access to 24 hours on-call Nursing Services as needed
Medication training and assistance
Specialized supervision to provide safety and support
Contract services by licensed and experienced consultants as applicable
Quarterly and annual reviews to update progress
