ELIGIBILITY

Men and women at least 18 years of age with a primary diagnosis of Intellectual Disabilities ranging from mild to severe.

Must possess or be eligible for ID Medicaid Waiver.

At least two deficits documented at the adaptive skill level in areas to include: Health, communication, task learning skills, personal/self-care, mobility, social skills, or community and home living skills.

Other disabilities such as hearing loss, spastic cerebral palsy, vision impairment, epilepsy, diabetes, autism, down syndrome, etc.
